PURPOSE:

To provide guidance got an employee seeking to apply for a leave of absence (LOA).

POLICY:

A benefits eligible employee, who has completed six years of continuous and active employment, is eligible to apply for a fully-paid leave of absence. An unpaid leave of absence may be applied for at any time and will be reviewed by management.

GUIDELINES:

1. To apply for an unpaid LOA, send an email to benefits@company.com with the following details. Please remember to copy your manager.

  • Name:
  • Employee ID:
  • Reason for LOA:
  • Start Date:
  • End Date:

Note: further documentation may be required to support the LOA request

2. To apply for a company sponsored LOA, send an email to benefits@company.com with the following details. Please remember to copy your manager.

  • Name:
  • Employee ID:
  • Start Date:
  • End Date:
